May cover a wide variety of conditions including (but not limited to): obesity, hypercholesterolemia, cardiovascular disease, diabetes mellitus, hypertension, kidney disease, eating disorders, gastrointestinal disorders, seizures (i.e., ketogenic diet), obstructive sleep apnea, anorexia nervosa, epilepsy and recurrent seizures, and other conditions (e.g., chronic obstructive pulmonary disease), pregnancy and postpartum. See this link.DOES NOT COVER nutrition counseling for asthma, attention-deficit hyperactivity disorder and chronic fatigue syndrome.Maryland Physician’s Care: follows CMS guidelines and covers only diabetes, renal conditions, high-risk pregnancy, patients under 21 with medically necessary condition and doctor referral.Johns Hopkins Priority Partners Plan: Diabetes, Pregnancy and postpartum, EPSDT (Early and Periodic Screening, Diagnosis, and Treatment for < 21 yr olds), Pre-Bariatric Surgery (2 visits), Post-Bariatric Surgery (1 visit), Chronic Renal Failure, Pre-ESRD (End-Stage Renal Disease), Weight Management (BMI of 30+), hyperlipidemia, coronary heart disease, hypertension or congestive heart failure, CHF -Newly diagnosed or new to the PCP), CHD - A cardiac event or its equivalent (e.g. stroke, PVD) with or without dyslipidemia. Other condition may be covered when prescribed by a doctor.Johns Hopkins US Family Health Plan (USFHP): Diabetes, Pre-Bariatric Surgery (2 visits), Post-Bariatric Surgery (1 visit), Ketogenic diets for treatment for epilepsy.Johns Hopkins Employer Health Plan (EHP): Pre-Bariatric Surgery (2 visits), Post-Bariatric Surgery (1 visit). All other conditions: 1 initial visit and one follow up visit. Additional visits are subject to review.Medicare: Diabetes, patients with kidney disease (but who are NOT ON DIALYSIS), or patients who’ve had a kidney transplant in the last 36 months. Medicare covers 3 hours for first year, and 2 hours for each following year.Riverside Health: covers high-risk populations such as obesity, hypercholesterolemia, cardiovascular disease, diabetes mellitus, hypertension, kidney disease, pregnancy and postpartum, EPSDT (Early and Periodic Screening, Diagnosis, and Treatment for < 21 yr olds).UnitedHealthcare (UHC): medicaid/community plan covers most conditions, but most commercial plans have no coverage.Tricare: does not cover outpatient nutritional counseling.CareFirst Blue Cross Blue Shield (PPO only): Diabetes (all types, including gestational), Malnutrition, Cancer, Kidney disease, Cardio- and/or cerebrovascular disease. Eating disorders, Hypertension, Gastrointestinal disorders, Seizure disorders, Hyperlipidemia, obesity Screening and counseling, Endocrine/metabolic disorders amenable to dietary therapy.
